Supporting Older Australians to Live Independently at Home

At SA Disability Care, we're committed to helping older Australians live safely, comfortably, and independently in their own homes for as long as possible. As part of this commitment, we are proud to offer services under the Support at Home Program, the Australian Government's new aged care model that will replace the current Home Care Packages (HCP) and Commonwealth Home Support Programme (CHSP) from July 2025.

This streamlined program is designed to make it easier for older Australians to access tailored care and support that meets their individual needs, right in their own homes.

Support at Home will bring together current in-home aged care programs, ensuring a simpler and more equitable system for older people that helps them to stay at home for longer. Support at Home will provide improved access to services, equipment and home modifications to help older people remain healthy, active and socially connected to their community.

What is the Support at Home Program?

The Support at Home Program is a new model of aged care that brings together different services into one flexible, personalised system. It's designed to reduce complexity, improve access, and give older Australians more control over their care.

Instead of separate programs, the Support at Home model offers one clear pathway for those aged 65 and over (or 50+ for Aboriginal and Torres Strait Islander people) to receive help with everyday tasks, health services, home safety, and social connection.

At SA Disability Care, we are already preparing for this transition and are ready to guide our clients through every step of the process.

What Services are Available?

Through the Support at Home Program, SA Disability Care will deliver a broad range of services tailored to each person's goals and support needs, including:

Daily Personal Care

  • Assistance with showering, grooming, and dressing
  • Help with mobility, transfers, and continence care

Domestic Support

  • Cleaning, laundry, and household tasks
  • Grocery shopping and meal preparation

Transport Services

  • Transport to medical appointments, community centres, or shopping trips

Health and Clinical Support

  • Allied health services such as physiotherapy and occupational therapy
  • Medication management and wellness support

Nursing Services

  • Medication administration and management
  • Wound care and dressing changes
  • Chronic disease monitoring (e.g. diabetes, heart health)
  • Post-hospital care and recovery support
  • Health assessments and care planning by qualified nurses

Home Maintenance and Safety

  • Garden and home upkeep
  • Small modifications (e.g. ramps, grab rails) for safety and accessibility

Assistive Technology

  • Equipment and tools to support daily living and independence

Social Connection

  • One-on-one social support
  • Group activities and community outings

Who is Eligible?

To be eligible, individuals must be:

  • 65 years or older (or 50+ for Aboriginal or Torres Strait Islander people), and
  • Have been assessed through My Aged Care as needing support to live independently at home.

How Does it Work?

1. Assessment – Contact My Aged Care on 1800 200 422 or visit myagedcare.gov.au to arrange an assessment.

2. Care Planning – Once approved, you'll work with a care provider to design a personalised care plan that aligns with your goals and preferences.

3. Flexible Service Delivery – You can choose the services you need and adjust them as your needs change.
