We offer a range of services and levels of assistance, tailored to your individual needs, goals and funding available in your Home Care Package (HCP). Our Care Plans are developed with you, your support network and any other health care professionals.

Home Care Packages (HCPs) help seniors to access affordable care services so that they are able to stay living in their homes as they age.
The packages are supported by the government and provide financial support towards help so you can remain living independently for as long as possible.
HCPs are open to all Australians requiring coordinated assistance in the home and assessments are free.
SA Disability Care is an approved provider of services under Home Care Packages Levels 1, 2, 3 and 4.
If you are not yet approved to receive a Home Care Package, we can assist you to navigate the assessment and application process.
When meeting with us for the first time, you should bring information on your income and relevant personal identification documents. If you have already been approved for a Home Care Package, you should bring your approval letter and support plan, as well as the outcome of your income assessment.
The income assessment helps to establish any co-contributions you may be requested to make and will give you a clearer understanding of your overall cost.
